Dr. Chawla’s practice membership includes:
- An annual comprehensive preventative health assessment and physical exam, including EKG as medically indicated
- Annual flu shot
- Up to 7 routine or sick visits per year
- Home visits as clinically indicated*
- Guaranteed same day or next day appointments
- Minimal in-office wait time
- Extended office hours around patient’s schedule and needs
- Direct access to Dr. Chawla by cell phone and email
- Hospital visits
- Careful coordination of medical care with specialists, therapists, and other medical personnel
* Policy Update: During the Covid-19 pandemic, home visits will be conducted at the discretion of Dr. Chawla. Home visits will be given only to fully vaccinated patients living in homes with fully vaccinated individuals.
Payment
The practice charges an annual fee of $1700, which covers the above listed services. Dr. Chawla will order all lab work and radiology tests at a facility that participates with your insurance. For patients requiring more than 8 total visits/year, additional visits will be charged $75 each.
